We are looking for a Community Health Worker to work directly with our patients to help them navigate their health and social needs. You will work with an assigned panel of members dealing with unmet social needs and numerous health conditions.Responsibilities
- Educate patients on disease prevention and healthy behaviors
- Cordinate comprehensive care by scheduling appointments and facilitating follow-ups
- Address social determinants of health by connecting patients with essential community resources
