On-site presence required; no remote option 24-hour call coverage with beeper call Locums will NOT be responsible for supervising other providers

Travel Reimbursement:

Credentialing Requirements:
To expedite the credentialing process, please ensure the following items are readily available for your provider at the time of acceptance: MD diploma Internship/residency/fellowship certificates All medical licenses DEA Board Certification Life support cards (BLS)

Current and previous certificates of insurance Driver???s license Professional color photo/headshot CMEs for the past 2 years Immunization records, including TB, Flu, HepB, Tdap, Varicella, and COVID References Case logs Submission Requirements:
Board Certified - Required Oregon license - Required DEA and CSR - Required BLS - Required Local candidates - Highly preferred COVID-19 Vaccination: Proof of COVID vaccine is required at the time of submission Must be submitted for consideration to work at this facility

Certification Requirements:
Board Certified Controlled Substance (CSR/CDS)
