- Location: Must be in the NY Metro area including NYC, Bronx, Brooklyn, Queens
- Responsibilities:
- Re-evaluates client functioning, health, and psychosocial status
- Performs visits to conduct assessments and establish care plans
- Interacts with diverse population and caregivers
- Requirements:
- Current NYS/NJ RN license and registration
- Bachelor's degree in Nursing preferred
- Minimum of one year experience in acute care or community health nursing
- Knowledge of Medicare, Medicaid, and NYS DOH standards
- Bilingual in English/Spanish, Chinese preferred
- Specialty: Case Manager / Utilization Review, Home Health, Skilled Nursing / Long Term Care
- Employment Type: Permanent
