    Description

    Job Title: Licensed Practical Nurse (LPN) - PRN

    A key member of the Northpoint team, the LPN provides and oversees medical services to patients.

    Essential Responsibilities:

    • Interact with patients, observe behaviors, and communicate significant observations to the treatment team as needed
    • Administer medications to patients and engage in education process
    • Utilize de-escalation skills when exposed to volatile patients to maintain personal safety for the patient and others
    • Provide admission, orientation, education, and discharge plans to patients and families to aid in the treatment process
    • Consult with the multidisciplinary treatment team in the development of treatment plans and discharge planning for excellent patient care
    • Maintain a clean, orderly, and safe environment, and report needs for repairs and maintenance to appropriate staff
    • Admit and discharge patients
    • Obtain vital signs, monitor symptoms, and document in the patient record
    • Comply with all laws, regulations, and scope of licensure
    • Monitor patients on an ongoing basis to continually assess needs and facilitate medical treatment and interventions
    • Document medical assessment, diagnosis, progress notes, roundings, and other treatment planning items in a timely manner for comprehensive and quality patient care
    • Actively participate as a member of the multidisciplinary treatment team and maintain effective working relationships with employees and peers
    • Maintain professional boundaries with patients at all times
    • Oversee and participate in quality assurance and compliance activities of the facility
    • Provide medical updates to utilization review and representatives of managed care to coordinate reimbursement of treatment services
    • Adhere to all company policies and procedures
    • Maintain confidentiality in accordance with established policies and procedures and standards of care
    • Other clinically appropriate services and special projects as assigned
