    Summary & Objective

    The Manager, Registered Nurse Field Case is responsible to assume primary responsibility for a patient caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process.

    Initiates communication with attending physicians, other staff members and other agencies as needed to coordinate optimal care and use of resources for the patient.

    Conducts accurate assessment of patient at SOC and throughout the episode of care.

    Essential Functions
    Completes nursing assessment forms in a timely manner per agency policy and procedures.
    Supervises Home Health Aides every two weeks, and LPNs per policy
    Identifies physical, emotional, and developmental needs of the clients
    Develops individual nursing care plan reflects current client needs prior to case conference
    Provides health education to the client and their families
    Makes proper and accurate clinical record documentation that reflects delivery and safe home health care services
    Prepare adequately for SOC by contacting patient/CG the day before discharge or when you receive the referral to confirm SOC date and time

    Other Duties
    Assesses client condition on every home visit.
    Communicates client assessments to appropriate individual-DPS/Clinical Manager/MD.
    Implements nursing interventions to meet client needs and changed conditions.
    Documents accurate clinical record on every home visit per agency policy and procedures.
    Accurately transcribes, implements, and evaluates the physician's plan of care in a timely manner.
    Implements infection control and safety measures per agency policy and procedures.
    Monitors client for compliance with defined plans of care.
    Assess, recommend treatments and treat wounds, performing dressing changes, wound vac therapy
    Initiation, teach and/or administration of home IV therapies; occasionally place IVs, change PICC line dressings.
    Insert and irrigate foley catheters.
    Suction and cleans tracheal tubes.
    Draws blood for laboratory testing.
    Complies with the home health agency's policy and procedures manual.
    Complies with federal, state, and local regulatory agencies.
    Provide other duties of healthcare team member.
    Perform other duties as assigned
    Supervisory Responsibility
    ~ May serve as an interim department leader depending on need

    Physical Requirements
    ~ Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

    Requirements
    Knowledge & Experience Requirements
    At least 1 year or more with current/active Florida State Professional License
    At least 1 year of more experience working with a Medicare Certified Home Health agency
    OASIS SOC Competency
    Knowledgeable of AHCA regulations, Medicare COP's guidelines
    Proficient with use of computers
    Ability & willingness to work a flexible schedule
    Excellent oral and communication skills
    Must have knowledge of computer office/clinical software
    Must be able to read, write and understand the English language
