    The Family Support Case Manager provides services to special populations, including refugees, asylees, and other ORR-eligible populations. This position will address barriers to client safety and wellness, and assist identified cases with stabilization by promoting self-sufficiency and long-term integration for clients and/or family members. The Family Support Case Manager will support these goals through ensuring the basic and specific needs of clients are met, including any unique needs from especially vulnerable populations.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    • Provides client-focused counseling and crisis-intervention support as needed, including benefit enrollment, health/mental health services, employment services, and English-language training.
    • Initiates service plans, and reviews and closes cases in a timely manner and in compliance.
    • Provides ongoing orientation and education on resettlement topics such as medical benefits, basic public health counseling, Medicaid, housing, and work etiquette.
    • Facilitates connection with trained mentors to eligible client population
    • Assists clients in accessing and connecting with appropriate external services and public benefits.
    • Maintains and updates database with client information, assessments, resources, and referrals.
    • Participates in program network meetings, stakeholder meetings and work with community partners to learn about resources for clients and make referrals when needed
    • Participates in in-service training and conferences related to the programs.
    • Maintains up-to-date documentation in client's case file to ensure services are properly documented throughout the client's service period.
    • Provide data required for various reports and submit all required reports in a timely and accurate fashion.
    • Collaborates with staff peers to ensure equity of service and proactive assistance for all clients.
    • Complies with all federal, state, and agency contracts.
    • Performs other duties as assigned.

    QUALIFICATIONS

    • Broad knowledge of practices, principles, and methods employed in the resettlement of refugees preferred.
    • Working knowledge and understanding of U.S. Immigration laws and regulations preferred.
    • Understands local mainstream community resources and how to access those resources.
    • Cross-cultural sensitivity and knowledge with the ability and desire to work with people of other cultures.
    • Outstanding organization, problem-solving, negotiation, decision-making, and mediation skills.
    • Computer skills in Word, Excel, and the use of email and other applications; written communication skills, ability to edit and the ability to make presentations.
    • Ability to explain facts and information to clients for who English is not a primary language.
    • Maintains punctuality for work, appointments and report dates.
    • Desires to innovate, and proactively seeks to respond to client needs with these innovations.
    • Possesses a valid New Mexico driver's license; has a reliable and consistently available vehicle; and is able to regularly travel throughout the agency's service area by way of interstate highways, and primary and secondary roads. Must carry automobile liability insurance at the level of $100,000/300,000/100,000.

    EDUCATION and/or EXPERIENCE

    Associates degree or 2 years of experience in a related field such as social work, human services, humanitarian, mental health or other related field. Equivalent combination of education and experience accepted. Fluency in a 2nd language is a plus.
