Full-time Remote Telehealth Palliative Care Opportunity - Texas, United States - Monogram Health

Monogram Health is seeking a Hospice Palliative Care physician for a full-time position in Houston Texas. Monogram serves patients with CKD, ESRD, and chronic conditions. MDs work from their home office. There is a 10% monthly travel requirement around the Houston area.

Roles and Responsibilities :
Remote (Work from home), Monday-Friday 8:30 am-5:30 pm
Treat and diagnose patients with CKD and Renal diseases
Serve as a collaborator with Nephrologist and other field clinicians
Apply evidence-based criteria in determining the appropriateness and necessity of care
Identify and make practical, pointed interventions to improve member care


Requirements :
Must be board-certified in Internal Medicine, Hospice, or Palliative Care
MDs must be licensed in Texas and live in the Houston area.
Must have experience in evidence-based clinical care
Must have experience in CKD, ESRD, and chronic care
Must have experience with quality improvement, risk adjustment, and medical utilization management
Must have strong management skills
