Job Summary:

Physical Therapist will provide coordinated care to clients of all age groups. Plans implements and evaluates client care plans to restore or maintain patient well-being. Plans organize and conducts physical therapy treatment based on the medical referral and evaluation.

Job Responsibilities:

· Promote /exemplify the Company mission, vision and values at all times.

· Perform clinical assessments; identifies patient needs and appropriateness of client.

· The document, prepare/revise and completes a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.

· Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).

· Plan and provide appropriate treatment for neuro-muscular, neurological and orthopedic disorders based on medical referral and therapy assessment.

· Assist/instruct client/family in transfer techniques appropriate to client abilities. Instruct on use of equipment.

· Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case conferences/Team Meetings.

· Monitors assigned cases to ensure compliance with requirements of third party payers.

· Completes appropriate documentation in a timely manner to assure compliance with company policy.

· Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.

· Meet mandatory continuing education requirements of the Agency/licensing board.

Requirements:

Experience:

Two (2) years' experience as a PT in a clinical care setting.
Preferred, Home health experience.

Home Health PT Requirements:

· Current State professional license as a PT in any/all applicable states

· Current CPR

· A valid driver's license and reliable transportation, insurance required.

· Therapist skills as defined as generally accepted standards of practice.

· Excellent observational and problem-solving, decision-making and report writing skills.

· Excellent interpersonal skills and ability to communicate effectively.

Education:

· Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.
