    North Star Family Center is a nonprofit organization foster care agency based in Fresno, California. Our center is dedicated to providing comprehensive support for children and families in need by promoting a safe and nurturing environment for children and parents.

    Role Description

    North Star Family Center targets and supports the children and non-minor dependents that have significant behavioral, medical, and therapeutic needs. These include but are not limited to violent offenses, property damage, sex offenses and perpetrators, aggression and assaultive behaviors, animal cruelty, Commercially Sexually Exploited Children and Youth, eating disorders, gang activity, habitual truancy, psychiatric hospitalizations, runaways, severe mental health issues, suicidal ideations and/or self harm, substance use and abuse, and frequent placement moves due to intensive behaviors.

    This is a full-time on-site role for a Case Manager at North Star Family Center. As a Case Manager, you will be responsible for assessing client needs, developing and implementing individualized care plans, connecting clients with relevant resources and services, monitoring client progress, and advocating for their rights and well-being. You will work closely with clients, other professionals, and community organizations to ensure the best possible outcomes for the families we serve.

    Qualifications:

    Language Skills:

    Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups, and/or boards of directors.

    Reasoning Ability:

    Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exits. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

    Education and/or Experience:

    1) A Bachelor's degree and six months of experience working with children who have serious emotional or behavioral needs or have special needs, including but not limited to intensive medical needs.

    2) An Associate's degree and one year of experience working with children who have serious emotional or behavioral needs or have special needs, including but not limited to intensive medical needs.

    3) North Star Family Center will consult the Department and ask for an exception if educational requirements are not met, but the applicant posses at least two years of direct experience working with children who have serious emotional or behavioral needs or have special needs, including but not limited to intensive medical needs. They must also have demonstrated a combination of education, skills, and experience to meet the needs of the target population including but not limited to cultural and linguistic needs.

    Certificates, Licenses, Registrations:

    • Must have and maintain a valid California Driver's License, maintain automobile insurance coverage and have access to an automobile.
