    Description

    We are seeking a dedicated and compassionate Pharmacist to join our team at CPMC-California Pacific Med Center Van Ness.

    Job Summary:

    The successful candidate will be responsible for delivering pharmaceutical care through the provision of patient-centered clinical services, medication information, education, medication preparation and distribution. They will exercise sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on diagnosis in a timely manner.

    Key Responsibilities:

    Pharmacy Services:

    • Preparation and Dispensing: Prepares medications by reviewing and interpreting physician orders, detecting therapeutic incompatibilities, and dispensing medications by compounding, packaging, and labeling pharmaceuticals.
    • Medication Control: Controls medications by monitoring drug therapies; advising interventions; and ensuring that routine and priority tasks are completed within established departmental time frames.
    • Mentorship and Training: Mentors and fosters a constructive environment that helps students, interns, and Pharmacy Technicians build confidence in their skills, knowledge, and abilities.
    • Pharmacological Information: Provides pharmacological information by answering questions and requests of healthcare professionals, counseling patients on drug therapies.

    Clerical Duties:

    • General Clerical Tasks: Performs general clerical tasks such as answering phones, updating patient information, or retrieving, entering, and revising patient Electronic Health Records (EHR).
    • Documentation: Prepares and completes accurate documentation related to patient information in medical and departmental records as required.

    Safety Protocols:

    • Infection Control: Participates in quality improvement processes, including updating and revision of safety and procedure manuals and interdisciplinary projects.
    • Work Environment: Maintains a clean, neat, and safe working environment by complying with policies, procedures, and regulations.
    • Confidentiality: Protects patients and technicians by adhering to infection-control protocols and maintaining strictest confidence of all patients Protected Health Information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure.

    Education and Certification:

    • Doctorate: Graduation from an accredited school of Pharmacy.
    • Certification and Licensure: PHARMD-Licensed Pharmacist OR PHARMR-Registered Pharmacist ACLS-Advanced Cardiac Life Support (for those who will be responding to codes).

    Experience and Requirements:

  • Typical Experience: 1 year of recent relevant experience.
  • Shift Hours: 8 hours per day.
  • Days of the Week: Variable.
  • Weekend Requirements: Rotating weekends.
