    Community Living Services (CLS) is an innovative and revolutionary healthcare model that allows adults with disabilities as well as seniors and adults with disabilities to live at home longer by transitioning them out of nursing homes and back into the community by partnering with health plans and Medi-Cal. This inventive, nationally recognized model is one of the most promising for seniors and adults with disabilities to date, transforming their lives by providing them with a higher quality and more independent life and saving millions of dollars for healthcare systems.     CLS is seeking a highly skilled and experienced Program Manager. The primary responsibility of this critical position is oversight of all programs within two very large counties; Riverside and San Bernardino. This includes programs undergoing implementation and newly identified expansion programs in the region. The Program Manager, will supervise Clinical Supervisors and other Clinical and Administrative Staff. Other key duties and responsibilities may include, but are not limited to:

    Program Development

    • Developing and implementing Enhanced Care Management and Community Supports program in collaboration with Medi-Cal Managed Care health plans and the IOA Riverside/San Bernardino Counties implementation team.
    • Directing an interdisciplinary, multi-agency team of clinical experts who are responsible for transitioning and supporting individuals out of long-term skilled nursing facilities into more independent settings.
    • Working in collaboration with Riverside and San Bernardino County providers to identify gaps in community-based services and supports for high-risk, low-income individuals.
    • Providing final approval for all community living plans prior to discharge
    • Working in collaboration with Medi-Cal Managed Care health plans to identify gaps in long term services and supports for individuals enrolled in Medi-Cal Managed Care
    • Becoming an expert in supportive housing options in Riverside and San Bernardino Counties; integrating Community Living Services into new and existing housing resources.
    • Leading and developing expertise in long term care facility discharge practices, capacity, and providers in Riverside and San Bernardino counties, developing relationships with leaders of all long term care facilities.
    • Participating in relevant meetings and workgroups within Medi-Cal Managed Care plans and County providers to develop a more integrated long-term care community support system.
    • Participating in local and statewide associations advocating on behalf of improved community-based long-term care services
    • Collaborating with Vice President of Community Living Services and the Vice President of Strategic Partnerships, identify additional funding opportunities to support the expansion of services in Riverside and San Bernardino Counties.
    • Working with other IOA Managers, Senior Directors, and VP of Community Living Services to develop best practice for community-based managed long-term support and services.
    • Working with Clinical Supervisors to develop short term and long-term programmatic goals and objectives.
    • Developing internal and external budgets, forecasts, and variance analyses
    • Participating in relevant Institute on Aging meetings and work groups to integrate program operations into the agency.
    • In collaboration with IOA Marketing Department, developing and presenting materials and outreach information to interested stakeholders in the community about IOA.
    • Partnering with IOA Talent Acquisition to develop pro-active recruitment plans to ensure necessary staffing levels are maintained
    Quality Assurance
    • In collaboration with the IOA Quality Improvement Department, establishing a quality improvement and assurance program with quarterly meetings to review outcome data.
    • In collaboration with the IOA Quality Improvement Department, designing and leading evaluation projects that assist in demonstrating program outcomes and value.
    • Providing program reports and outcomes data as needed.
    • Responding to staff and client grievances as needed
    Finance
    • Developing, implementing and managing regional budgets.
    • In collaboration with Vice President of Community Living Services, developing financial metrics that demonstrate cost savings achieved by programs.
    • In collaboration with Vice President of Strategic Partnerships, identifying and securing additional funding streams for programs intended to support seniors and people with disabilities living in the community.
    • Performing other duties as assigned or required.
    REQUIRED QUALIFICATIONS
    • Master's degree or higher in Administration, Public Health, Social Work, Nursing, Psychology, Counseling, Rehabilitation, Gerontology Sociology, or a Master's degree in a comparable or related discipline from an accredited college or university
    • Minimum 2 years supervisory experience in a medical or community-based setting
    • Experience in building and leading an efficient and effective multi-disciplinary team preferred.
    • Broad understanding of community-based health and social services and pertinent policy issues
    • Excellent interpersonal, verbal, and written communication skills
    COMPENSATION:

    Range: $95,000-$112,000

    This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.
