    * *Position:* Respiratory Therapist
    * *Location:* Pewaukee, WI
    * *Hours: *Monday-Friday, 8am-5pm. Weekends/On-call: as determined by leadership
    * *Requirements: *holds the following license; LRT, BLS

    *Sign On Bonus $3,000*

    *THE POSITION:*

    * Uses analytical ability to perform initial patient assessments including physical, functional, social and environmental resource requirements and needs. Ensures communication and coordination of services /care among all clinical services.
    * Performs initial equipment set-up and patient/family/caregiver education.
    * Develops patient specific plans of care based on initial assessments to include problems, goals and interventions.
    * Reviews and revises care plans on an ongoing basis and ensure that all patients are followed according to that plan per PHS policy.
    * Ensures that all required documentation is completed accurately and within the time frames set forth in PHS policy.
    * Participates in care conferences and collaborates with all care team members internally and externally.
    * Participates in day to day trouble shooting. Performs all on-call activities, analyze problems, formulates solutions, and notifies manager on-call as needed.
    * Attend department and staff meetings as necessary.
    * Schedule all patient visits per PHS follow-up policy.

    *THE QUALIFICATIONS:*

    * Certified Respiratory Therapist (CRT) by The National Board of Respiratory Care (NBRC)
    * Current Basic Life Support (BLS) certification required
    * One to two years of pediatric experience as a Respiratory Therapist

    *Preferred Qualifications:*

    * Neonatal & Pediatric Specialist (NPS)
