Qualifications:

Required:

Bachelor's degree or higher
Over 1 year of Home Health experience
ALL of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- Physical Therapy in New Jersey (NJ)
Full-Time position

Preferred:

Over 2 years of experience in ANY of the following:
- Community-Based Care
- Clinical Setting

Overview:

Our client is seeking a full-time Physical Therapist in Ocean County, NJ, to provide home care therapy to individuals, helping restore function, improve mobility, and relieve pain.

The ideal candidate will have:

A Bachelor's Degree in Physical Therapy
1 year of experience in a homecare setting
Ability to work well under pressure with strong attention to detail.

This role offers a dependable and compassionate work environment with opportunities to gain experience and become an invaluable resource to our clients.

Key Responsibilities:

Perform initial assessment or admission to the agency to determine patient treatment needs.
Develop a plan of care with measurable goals and specific discipline-related treatments.
Provide direct physical therapy services as outlined in the plan of care.
Re-evaluate and revise plan of care as needed with physician approval.
Facilitate total care services to patients by making appropriate referrals to other disciplines.
Submit all documents promptly.
Obtain physician approval to modify a current plan of care or treatment program.
Perform other related duties as assigned.

Schedule and Shift Details:

8:30 a.m. - 4:30 p.m.
1 weekend per month
Rotation of all national holidays.
