    GENERAL DESCRIPTION OF POSITION
    The Program/Project Manager (HIV) is a full-time, salaried position under the Outreach department of North Country HealthCare. The Program/Project Manager (HIV) will oversee all HIV/AIDS program at North Country including the management of service delivery processes and systems for Ryan White clients, support HIV clinical care efforts, and facilitate outreach education and testing activities for the organization. The service area includes Coconino, Yavapai, Mohave Counties including the Hopi and Navajo Nations. The position requires strong organizational skills, leadership and coordination skills, verbal communication skills, and flexibility. An understanding of the HIV/AIDS population, barriers to care, disease prevention and transmission, and sensitivity to the social stigma that exists for persons living with HIV are helpful.

    ESSENTIAL DUTIES AND RESPONSIBILITIES
    1. Manage, implement and evaluate the two Ryan White HIV Care and Services grants, the Health Choice Integrated Care screening and education grant, and a Mohave County grant for HIV care in northern Arizona.
    2. Oversee medical and non-medical case management and the provision of core medical and support services to clients living with HIV.
    3. Fulfill grant requirements for grant sponsors including scope of work implementation, tracking program expenses, managing the program budget and submitting required monthly, quarterly and annual reports, and grant renewals.
    4. Evaluate program quality and direct program improvements based on the Clinical Quality Management Plan with feedback from the Clinical Quality Advisory Board (North Country's CQI Committee).
    5. Enhance and develop relationships with county health departments, Arizona Department of Health Services, Clark County Department of Health, North Country satellite clinics, Tribal health providers, AIDS Service Organizations and other organizations/programs serving persons living with HIV in the service area.
    6. Maintain relationships with Steward Health Choice and Behavioral Health Homes implement the HIV/HCV Screening and Education Program.
    7. Oversee planning, implementation, and evaluation of an outreach HIV Screening Program.
    8. Supervise Ryan White Program Coordinators, Medical Case Managers and Program Specialist/Health Educators.
    9. Facilitate, organize, and utilize systems for tracking client services and program budgets.
    10. Maintain accurate and complete client files and program databases.
    11. Develop and review systems and protocols to screen clients for program and service eligibility.
    12. Maintain oversight of the monthly HIV Support Groups in the community.
    13. Promote North Country HIV services to organizations and groups.
    14. Travel throughout northern Arizona service area and to other meetings as necessary.
    15. Regular and predictable attendance.
    16. Continuous learning in both personal and professional development.
    17. Consistent review, comprehension and reply of company email and related correspondence.
    MINIMUM QUALIFICATIONS
    1. Three or more years of Public Health or Social Science experience.
    2. Three or more years of supervisory and budget experience.
    3. Bachelor's degree in a related field.
    4. Must have excellent organizational skills, as well as verbal and written communication skills.
    5. Strong data analysis and computer skills, Outlook, Excel and Word required.
    6. Must be able to work independently, and manage multiple priorities.
    7. Competent working with people of other ethnic backgrounds, low incomes, and in rural areas.
    8. Must be able to maintain strict client confidentiality.
    9. Ability to travel.
    10. Able to show flexibility when policies change or with client requests.
    SALARY

    $50,000.00/YR to $54,000.00/YR DOE
