Patient evaluation diagnosis treatment and management of various conditions providing urgent care services managing value-based patients utilizing HEDIS measures to evaluate improve patient care collaborating with interdisciplinary teams educating patients on maintaining proper health preventive care managing chronic conditions participating in quality assurance activities staying current with evidence-based practices monitoring performance metrics related to HEDIS value-basedcare implementing improvement strategies as needed medical degree valid medical license DEA license CPR certification minimum 2 years experience in primarycare setting preferred experience managingvalue basedpatients utilizingHedismeasuresforqualityimprovement strongclinicalanddiagnostic skills excellentcommunication interpersonal skills proficiencywithEMRsystems abilityto work effectivelyinfastpacedpatientcenteredenvironment bilingual EnglishSpanish is highlyfavorable demonstratedabilitytoprovidecompassionatepatientfocusedcare
