    Description
    The Behavioral Health Specialist, Subject Matter Expert (SME) is responsible for identifying, implementing, and managing cross-setting quality improvement strategies within hospital, nursing home, home health, hospice, dialysis, and physician practice settings. The Behavioral Health Specialist, SME works with the senior leadership, direct care and performance improvement professionals in these settings to provide technical assistance in the use of workflow redesign, team building, communication skills and emergency preparedness to ensure the implementation and ongoing internal monitoring of evidence-based behavioral health interventions, trauma-informed care and workplace resiliency in a timely and effective manner. The Behavioral Health Specialist, SME also works with professional health care organizations and community-based organizations to enlist their support as partners in improving transitions of care for individuals with a behavioral health diagnosis across the health care continuum.

    Duties include but are not limited to:
    • Recommend and implement strategies to recruit and engage stakeholders concerned with behavioral health, including health care providers, community-based organizations, faith-based organizations, community centers, human service agencies, political leaders, business leaders, media, consultants, educational institutions, and Medicare beneficiaries in collaborative partnerships to identify opportunities to improve patient outcomes related to quality and efficiency in transitional care.
    • Develop and lead the presentation of instructional curriculum and content for instructor-led and self-paced learning modules in various formats requiring minimal editing. Lead collaboration with other disciplines to analyze and incorporate participant feedback from prior events and utilize adult learning principles in the development of content and materials related to healthcare emergency preparedness.
    • Serve as a mentor or resource to Behavioral Health peers, new Behavioral Health specialists, or other assigned team members (participates in orientation, a resource to questions, provides feedback/suggestions to support work).
    • Provides ongoing 1:1 or group technical assistance and consultation to providers for all aspects of performance improvement, targeting improvement in care coordination, cross-setting communication, and patient/caregiver self-management skills related to behavioral health, establishing trust and the confidence of providers.
    • Assesses the quality improvement training and information needs of clinicians, administrative, and ancillary staff and coordinates appropriate educational efforts for these individuals and groups.
    • Works closely with the Healthcare Quality Improvement Programs (HCQIP) data analyst to design reports required to monitor provider and department performance, making necessary adjustments in interventions and plans in response to findings.
    • Attends and participates in IPRO regional conferences, department meetings, and professional education meetings.

    QUALIFICATIONS:
    • Strong communication skills (verbal and written) with the ability to communicate quality improvement data and methods to health care providers and community stakeholders with varying technical and clinical, and health literacy backgrounds.
    • Knowledge of the principles and practices of health care quality improvement and community organizing. Experience with quality improvement research techniques and knowledge of disparities in the care of underserved populations.
    • Strong interpersonal skills with a proven record of developing productive relationships with health care provider staff, consumers and community stakeholders including business and political leaders.
    • Strong time management skills with the ability to multi-task and meet multiple deadlines.
    • Proven record of success in promoting improvement in community health quality and efficiency.
    • Ability to work independently with minimal supervision, as well as in a team environment.
    • Ability to travel to provider sites and conferences as required.
    • Computer proficiency with a developed knowledge of Microsoft applications (Word, Excel, PowerPoint).
    • Ability to provide formal and informal presentations to internal staff and external customers.
    EDUCATION & EXPERIENCE:
    • Bachelor's degree in psychology, geriatric or psychiatric nursing, social work, or mental health counseling, required. Master's degree preferred.
    • Two (2) years of relevant experience working in a health care organization, such as a hospital or nursing home, required.
    • Prior experience leading interdisciplinary teams to improve behavioral health initiatives, required. Experience working with community coalitions and/or behavioral health readmissions reduction preferred.
    • Certified Professional in Healthcare Quality (CPHQ) or other quality certification preferred.
    • Training in or experience with cultural competency and/or health literacy preferred.
    The salary range listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. The annualized salary range for this position is $80, $85, Actual salary depends on a variety of factors including experience, education, credentials, location and budget.
