    We are searching for a talented Supportive Care Ops Coordinator

    Hours:
    Work 24hrs, paid for 32hrs

    Shift:
    Day shift; 8:00am - 4:30pm; Saturday, Sunday, and Monday

    Location:
    Southcoast Business Center - Fairhaven, MA

    Travel:
    Driving on average 3 or more times per week required

    Responsibilities


    Under the general supervision and direction of the Director, Supportive Care Services or designee, oversee the daily operations of Supportive Care Services.

    Responsible for daily management of all aspects of patient scheduling, including processing of referrals, case assignments, prioritizing of visits and clinician assignments and coordination of admissions.

    Manage administrative functions such as medical supplies and medications, clinically triaging incoming patient calls and physicians orders. Function as a liaison between the patient, referral source, intake staff, and clinical team.

    Qualifications


    • Current LPN licensure in the Commonwealth of MA is required.
    • CPR and Basic Life Support (BLS) certifications are required.
    • Supervisory experience, preferably in a Hospice setting, is desired.
    • Must possess and demonstrate highly effective interpersonal, customer service and communication skills.
    • Proficient computer skills, including experience with various software, database and electronic records programs is required.
    • Five (5) years of nursing experience, preferably in Homecare or Hospice required.
    • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hire
    Pay Range

    USD $ USD $40.85 /Hr.
