Setting: Hospice/Home Health

Location: Great Bend, KS

Schedule: Full-time days Mon-Fri, standard office hours with occasional evenings

The Social Worker role involves providing psychosocial services to patients, families, and caregivers:

• Offering group counseling, education, family conferences, individual counseling, and support sessions
• Evaluating patients' psychosocial distress and assisting them with understanding the emotional, psychosocial, and financial aspects related to home health and hospice care
• Conducting psychosocial reassessments as necessary, managing patients' psychosocial needs, coordinating with the community, planning interventions with the treatment team, and utilizing community and government resources

REQUIREMENTS:

• Master's degree in social work from an accredited School of Social Work
• LMSW or LSCSW licensure with the Kansas State Board of Social Work
• Familiarity with social work principles and techniques, and adapting standards of practice for specific medical cases
• BLS certification upon hire or within 2 weeks
• Open to new graduates; supervision and training will be provided if necessary
