    Clinical Consultant Social Worker- Hospice

    • Ensure organizational excellence in customer service and quality of care while promoting cultural experience at assigned hospital.

    This position is responsible for providing services to patients, families and significant others including:

    1. Completion of biopsychosocial assessments
    2. Psychiatric diagnoses
    3. < li>Treatment interventions < li,Therapy sessions (individual, couple, families or group)< p > Receives referrals for individuals from at-risk populations from interdisciplinary team members . Provides crisis intervention , information , referral , linkage to community resources as needed . Develops plan of intervention based upon assessment to resolve identified problems ; supports enhance biopsychosocial functioning through individual family or group modalities employing clinical intervention skills . Coordinates management patient financial outcomes assigned population works collaboratively with interdisciplinary staff external organizations timely cost effective management patient care monitors ensures completeness all assigned patients areas treatment plan reports thoroughly documents assessments interventions accordance department standards maintains awareness current hospital appropriate care options assist client performs additional functions may be necessary position
