    To be eligible for this position, you must meet the following qualifications:

    • Graduate of an accredited LPN/LVN school of nursing
      • ">
    • National Provider BLS - American Heart Association within 30 days of hire
      • ">
    • Oregon Licensed Practical Nurse upon hire
      • ">
    • Driving may be necessary as part of this role; caregivers must comply with state laws and requirements
      • ">
    • Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
      • ">
    • In compliance with Providence St. Joseph Health - OR, criminal background check upon hire
      • ">
    • 1 year of experience in nursing: long term care, acute care, ambulatory care, or home health and hospice care
