Description
Gratiot Integrated Health Network (GIHN) has an immediate opening for a full-time, Court and Jail Services Therapist. The Court and Jail Services Therapist is responsible for providing therapy services, annual assessments, determining the appropriate treatment plan, and making referrals to other services if deemed necessary, to consumers involved in the court system, lodged in the county jail, and/or receiving mental and/or co-occurring health services. Essential Functions.

1. Performs annual assessments of consumers receiving mental health and co-occurring services to determine individual and/or family needs. This includes diagnostic evaluation; obtaining personal, family, emotional, social and medical information; and crisis intervention. Also responsible for monitoring treatment plans to assess consumer progress and making adjustments to goals, when appropriate.
2. Involved in organized, on-going, collaborative efforts that involve individuals with co-occurring mental health and substance use disorders which may require services from multiple systems.
3. Provides on-location service delivery to assigned consumers and their families, and non-GIHN consumers lodged in the county jail.
4. Provides crisis intervention services to referred consumers, open consumers, and non-consumers lodged in the county jail by phone, walk-in, appointment basis, or jail request.
5. Serves as designated jail services therapist and works collaboratively with the jail administrators to provide behavioral health services to inmates, including individual and group therapy, crisis services, case management, and mental health assessments to inmates.
6. Demonstrates core clinical skills in treating consumers with co-occurring disorders using evidence based, welcoming, motivational interviewing and stages of change, practices.
7. Performs diagnostic evaluation in collaboration with the consumer's physician, GIHN's Access Department, and/or the Recovery Program Supervisor.
8. Provides collaboration with referral sources such as physicians, schools, community professionals, other agencies, hospitals, court personnel and law enforcement, to ensure continuity of care and services.
9. Enhances community understanding and acceptance of individuals with a severe, persistent mental illness, co-occurring disorder or developmental disability, and existing GIHN services, through educational community presentations throughout Gratiot County.
10. Collaborates effectively with other healthcare providers to provide integrated care for GIHN consumers.
11. Able to accurately diagnose mental illness, substance use disorder, psychological disturbance and potential developmental disability, using approved diagnostic nomenclature.
12. Effectively conducts therapy with children, adolescents and adults utilizing individual, group, family or other appropriate modalities of treatment, to ensure that the least restrictive procedures and shortest length of stay in treatment is achieved.
13. Demonstrates competency working with children and/or adults, in targeted populations, who have experienced trauma and/or sexual abuse.
14. May serve as a member of the emergency services crisis team, providing 24 hour crisis intervention for the residents of Gratiot County.
15. Maintains complete, detailed and timely records, in compliance with regulatory and professional standards, GIHN policy and third party funding source requirements.
16. Maintains the security, confidentiality and integrity of consumer's clinical documentation in a manner that is consistent with HIPAA, MDHHS and CMH policy and procedure.
17. Adheres to GIHN and professional code of ethics.
18. Participates in staff meetings, patient review meetings, the clinical record review process (if required), and in-service training as an effective member of the clinical services team.
19. Protects the security, confidentiality and integrity of all protected health information in a manner that is consistent with HIPAA, MDHHS and GIHN policy and procedure.
20. Assures consumers' rights are protected and recipient rights procedures are followed throughout GIHN and its provider network.
21. Attends and actively participates in GIHN required training as well as seeks external education opportunities for professional growth and development.
22. Abides by all applicable GIHN policies and procedures.
23. Maintains a safe and clean work environment.
24. Performs work and other duties as assigned by the supervisor.

Must demonstrate sensitivity, knowledge and use of appropriate approaches, skills and techniques which reflect an understanding and awareness of trauma and social, cultural and economic diversity of the target population served. GIHN employees will actively support the goal of person centered planning and integrated care through the recovery process and will acknowledge and develop competencies to support the understanding of the complexities of the human condition and the manner in which physical health, substance use disorders and physical and/or emotional trauma are interrelated to the need for mental health services. Must demonstrate computer skills, including entering and retrieving data. Must be able to read and interpret statistical data. Must demonstrate high level of interpersonal skills to handle sensitive and confidential situations and documentation and the ability to maintain a high level of confidentiality. Must exhibit strong spelling, grammar and both oral and written communication skills.
Supervisory Responsibility.
This position has no supervisory responsibilities.
Work Environment.
This job operates in a professional office environment; however, it requires travel to other locations including, but not limited to, jail, consumer's homes, physician offices, court houses, etc. This role routinely uses standard office equipment such as laptop computers and smartphones.
Physical Demands.

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop; kneel, crouch or crawl; talk or hear; and taste or smell. The employee must occasionally lift or move up to 20 pounds.
Position Type/Expected Hours of Work.
This is a full-time, salaried, exempt position. Some flexibility in hours is allowed but the employee is expected to be available during the core works hours of Monday through Friday, 8 a.m. to 5 p.m. the majority of the time, and must work 40 hours each week to maintain full-time status, unless an adjusted schedule is pre-approved (a minimum of 30 hours per week is required to maintain full time benefit status). Some evening hours and weekends may be required.
Travel.
Travel may be necessary for this position but is generally located within Gratiot County. A valid Michigan Driver's license is required to operate GIHN vehicles or to transport consumers in personal or GIHN owned vehicles.
Required Education and Experience.
Master's degree in Social Work, Professional Counseling or related field, is required. Michigan licensure appropriate to professional degree, i.e. LPC, LLP, LMSW, is required. Must have experience in the diagnosis and provision of services to individuals with mental health illness, developmental disabilities, substance use disorders and co-occurring disorders in a mental health or other human services agency. Must have QMHP or QMRP and a minimum of one year related work experience. Individual Specialized Training qualifies for a QMHP/QMRP. Additionally, must have CAADC certification or be willing to become certified upon employment.
Preferred Education and Experience.
Three years of work experience in the mental health field working with adult co-occurring disorders in the jail/prison population.
