    Description
    *MDS Coordinator – RN or LPN*Autumn Court, Long Term Care Facility, is seeking an experienced and skilled MDS Coordinator to join our team. We offer competitive pay, great benefits, and flexible schedules to ensure that our employees are happy and fulfilled in their roles.


    As an MDS Coordinator, you'll be responsible for coordinating the development, completion, and timely submission of MDS assessments for our residents.

    You'll also be responsible for ensuring compliance with all state and federal regulations related to MDS assessments and the Patient-Driven Payment Model (PDPM).

    The ideal candidate for this position will have previous PDPM experience and be an RN or LPN.

    To be considered for this position, you must have a valid nursing license and experience in MDS coordination. You should also have excellent organizational skills, be detail-oriented, and have strong communication skills.
