Performs duties such as bathing dressing assistance in nutrition and hydration locomotion ambulation feeding incontinence care toileting oral hygiene skin care grooming such as comb brush hair nail care shave Reports all cases of alleged violations involving mistreatment neglect or abuse including injuries of unknown sources Documents resident care accurately and completely at point of care utilizing computerized medical record system Notes resident changes of condition and reports same to LPN RN Answers call bells in timely polite manner Assists in provision of post-mortem care Performs vital signs as directed Reports on duty listens to or obtains report from Charge Nurse receives assignment Makes two hour prn rounds on all residents Keeps unit neat clean straightens nourishment station clean dirty utility room other areas assigned Ensure the resident is out of bed dressed ready to participate in activities choice Provide assistance with ADLs during activities if necessary Assist resident to participate individual activities choice set up equipment positioning assistance providing sufficient lighting space.Job description
