The Case Manager Assistant works closely with the case manager to ensure the discharge process is smooth and has the responsibility for facilitating discharge services,discharge appointments, and DME needs.
Responsibilities
- Complete documentation and communication requirements for post discharge services
- Works with DME companies to ensure all required equipment is ready for patient at discharge.
