    Health Center - Transportation Certified Nursing Assistant (CNA) M-F 8-4 (Full-Time, Charlton Mass)

    POSITION SUMMARY:

    The Certified Nursing Assistant promotes quality of life and a home environment by knowing the residents and from the framework of that knowing providing assistance with activities of daily living, and other direct care to residents as outlined in the interdisciplinary care plan, guided by the facility policies and under the direction of a licensed nurse.

    PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:

    The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

    1. Assist Health Center residents to and from appointments at the direction of the Transportation Coordinator
    2. Participate on Nursing units when not transporting Residents
    3. Recognize the autonomy and dignity of all residents.
    4. Communicate a sense of caring, concern, and dignity for residents.
    5. Understand how to place decision-making in the hands of the residents whenever possible and appropriate.
    6. Make prompt and accurate judgments with regard to resident care and emergencies.
    7. Work and communicate effectively as part of the unit team.
    8. Serves on facility committees and work teams as assigned.
    9. Participates in interdisciplinary care plan meetings.
    10. Maintain all aspects of confidentiality regarding the residents and the facility.
    11. Know and follow facility policies and procedures.
    12. Use resources with consideration to cost.
    13. Assist Staff with Resident Care, such as bathing, combing hair, brushing teeth, shaving, dressing, ambulation, and toileting and laundry as needed. 14. Records and documents care provided to the resident
    14. Obtain routine lab samples.
    15. Help residents with equipment for their treatment and care
    16. Help residents to move, transfer from bed to chair, or transport them within the building or on the grounds.
    17. Follow all infection control policies.
    18. Observe the resident and surroundings and remove any potential hazards which could cause accidents.
    19. Report to the nurse in charge all information and observations about the residents in their care

    QUALIFICATIONS:

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.     Educational Requirements and Experience:

    • Must possess a current Certified Nursing Assistant in accordance with the Massachusetts State regulations.

    Knowledge, Skills and Abilities:

    • Language Ability: Must be able to read, write, speak, and understand the English language.
    • Mathematical Skills: Must have basic math skills.
    • Cognitive Demands: Must possess the ability to make independent decisions when circumstances warrant such action. Must function independently and have flexibility and personal integrity.
    • Computer Skills: Must have computer literacy in Microsoft products, and MAR/TAR software proficiency.

    Competencies:

    • Good judgment and decision making.
    • Communication skills including the ability to communicate with residents, families and team members.
    • Reliability including reporting to work as scheduled and on time, and completing assignments according to facility policy.
    • Patience, tact, and a willingness to take direction and to work as a team member

    Environmental Adaptability

    • Work primarily indoors in a climate controlled setting.
    • Possible exposure to unpleasant odors
    • Possible exposure to chemicals as identified in the MSDS Manual
    • Possible exposure to infections, diseases and bloodborne pathogens
    • Continuous exposure to residents who are ill, confused, irritable and irrational
    • Subject to hostile and emotionally upset family members, personnel and visitors
    • May work beyond normal working hours and on weekends and holidays when necessary.
