    OVERVIEW OF POSITION:

    This position provides counseling and case management services to opiate addicted patients in accordance with licensing requirements, accreditation, and our clients guidelines. These services must be ethicall and consistent with state and federal guidelines and in accordance with requirements and guidelines of our clients-'s Services and other accreditation agencies.

    PRINCIPLE RESPONSIBILITIES:

    Perform walk-in intake screening and phone assessment screening.

    Process all new, transferring and/or guest dosing patients required intake paperwork.

    Thoroughly explain program rules, policies, guidelines and expectations to all new, transferring and/or guest dosing patients.

    Thoroughly explain to new, transferring and/or guest dosing patients the consequences of non-compliance with program rules, policies, guidelines and expectations of patients.

    Process all necessary paperwork for current, temporary transferring and/or guest dosing patients. (Example: release forms ,take-out requests, guest dosing requests, etc.)

    Maintain updated paperwork in all patient files to comply to with accreditation, Federal and State regulation.

    Maintain all patient records in the format setup by our client's services.

    Effectively manage assigned caseload.

    Maintain one-to-one counseling with each patient twice (2) per month to comply with the minimal state requirement.

    Maintain a direct face to face contact rate of60 % of hours worked.

    Counselors must ensure each patient keeps their appointments as stated in programs rules.

    Facilitate family meetings ,as needed by patient and requested by the organization.

    Facilitate (1) group meeting per week.

    Develop and maintain updated treatment plans and progress notes.

    Monitor patients' urine specimen results and address results with patient.

    Evaluate patient records for quality assurance in accordance with accreditation.

    Communicate effectively in both written and verbal with coordinating agencies on behalf of the patient and the organization.

    ADDITIONAL RESPONSIBILITIES:

    Participate in team building activities

    Attend and participate in staff meeting sas required

    Assist in emergency situations (example: evaluating impaired patients, calling 911, getting first aid and/or spill kits)

    Educate patients about opiate addiction

    Assist in other duties as assigned EDUCATION/TRAINING:

    Master's degree required in behavioral health field with clinical application

    Clinical credentials CADC / CAADC preferred, developmental plan required

    Up-to-date knowledge of addictions, HEP-C,HIV issues

    Knowledge and ability to use computers and software to include word- processing and database management.

    CHARACTERISTICS/EXPERIENCE PREFERRED:

    Knowledge of screening/assessments instruments preferred.

    Ability to communicate effectively, both orally and in writing preferred.

    Ability to work independently and as part of a team.

    Creative problem solver who can suggest ways to enhance program preferred.

    Ability to plan, organize, set objectives, time frames and prioritize preferred.

    Demonstrate experience in addiction and needs identification preferred.
