- Maintain regular communication with case managers/discharge planners and units to ensure timely follow-up on patient referrals.
- Collaborate with healthcare team members to address inquiries about homecare/hospice and community-based services for patient discharges.
- Coordinate care for clients transitioning home from the hospital who have chosen our services.
- Document initial assessment of homecare needs and potential service plan.
- Arrange services based on admission criteria, client care needs, and preferences.
- Secure physician orders for homecare/hospice services initiation.
- Use interviews, observations, and evaluations to assess clients and make recommendations on appropriate services.
- Assist in quality assessment and improvement activities within the branch.
- Identify, develop, and implement new programs/initiatives to meet client needs.
