Alta Health Services is excited to announce a PRN Physical Therapist position for our home health division in beautiful Salt Lake City, UT.

The following information provides an overview of the skills, qualities, and qualifications needed for this role.
Our Physical Therapist will help our patients with the following:

Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of our patient's physical and psychosocial functioning.

Responsible for planning, evaluation, and treatment implementation of assigned caseload.
Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.
We are looking for a PRN certified Physical Therapist for our home health care division.

Our Physical Therapist will enjoy a competitive hourly rate of $50-$55 as well as a per visit rate ranging between $70-$78.

Our Physical Therapist will need a reliable vehicle, current auto insurance policy, and a valid driver's license. 00 per hour
