- Provides direct patient care as defined in the state Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (OT, PT, MSW, SLP) to the registered nurse and/or Administrator/DON
- Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
- Participates in paid rotational clinical on-call (minimal)
- Participates in once weekly case conference
- Agrees to occasionally assist with CNA visits, in the absence of CNA
- Graduate of an accredited practical nurse or vocational nursing program.
- Three (3) years nursing experience/ preferred. Community/Home health experience is preferred but, willing to train the right candidate.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of Colorado or possesses a compact licensure.
- Possess and maintain valid CPR certification.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication and organizational skills.
- Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
- Must have reliable transportation and company-required auto liability insurance.
- Willing to travel within 20 miles radius.
