- Coordinates health and social services for older adults in Naturally Occurring Retirement Communities (NORCs) to enable them to age in their own homes.
- Collaborates with site staff to promote self-care management strategies among residents.
- Provides health-focused outreach, promotes healthcare activities, and makes referrals as needed.
- Serves as a liaison between healthcare systems during transitions and assists residents with connecting to appropriate care services.
- CPR/BLS certification requiredEducational Requirements:
• Associate's Degree in nursing required
Bachelor's Degree in nursing preferred
Work Experience:
Required Qualifications:
Minimum of two years working as an RN in LHCSA or CHHA.
Experience working with aging populations and providing clinical assessments.
