Job summary
We are seeking a Physical Therapist to join our team at AdvantageCare Rehabilitation in Emporium PA. As a Physical Therapist you will conduct comprehensive evaluations of patients physical conditions functional limitations and mobility develop individualized treatment plans based on clinical assessment and patient goals utilize various therapeutic techniques including exercises manual therapy and modalities such as ultrasound electrical stimulation heat/cold therapy maintain accurate timely documentation of patient evaluations treatment plans progress notes discharge summaries communicate effectively with other healthcare professionals patients families regarding treatment plans progress discharge recommendations educate patients families on therapeutic process home exercise programs injury prevention provide support encouragement to enhance motivation compliance therapy work collaboratively interdisciplinary teams ensure comprehensive patient care participate team meetings case conferences discharge planning coordinate care adhere regulatory facility policies including documentation standards safety protocols infection control procedures participate quality improvement initiatives contribute maintaining high standards patient care all duties assigned.
