    Physical Therapist-Home Care

    This position will focus primarily on the admissions/start of care visits.

    Responsibilities:


    Under the direction of the home care manager, the main function of the Physical Therapist is to organize, conduct, and evaluate prescribed therapy program of patients in a home health care setting.

    This position will focus primarily on the admissions/start of care visits.


    ESSENTIAL FUNCTIONS:
    Gather and review patient information and complete initial evaluation of patient

    Monitor and assess the effectiveness of treatment program and make changes in response to patient's progress, guiding patient toward goal achievement.

    Applies diagnostic and prognostic muscle, nerve, and joint functional ability tests.
    Collaborate with other health professionals, patient, and/or family in developing plan of care. Prepares progress reports.
    Instructs patient, family and/or caregivers in techniques to be continued after discharge.
    Manages work and patient schedule to make optimum use of time. Adjusts schedule to accommodate unexpected occurrences and to assist others as needed.
    Ensure accurate and complete documentation in the medical record. Maintain compliance with standards.


    Qualifications:
    State of Maryland License in Physical Therapy
    Current CPR Certification
    Prior experience in home health preferred
    Valid driver's license and reliable transportation

    This position requires daily travel in assigned region
