    7:00 A.M - 3:30 P.M

    Job Description

    Purpose:

    The patient escort is an integral member of the physical therapy division and receives assignments from the Director of Physical Therapy/designee in accordance with the operational procedures of the physical therapy division. The patient escort transports patients to and from the physical therapy division for treatment and will assist with other transport duties as required.

    Specific Duties and Responsibilities:

    • Obtains escort assignments for off ward inpatients from the PT Director/designee, which includes the patient's name, medical record number, ward, and the name of the therapist assigned to the patient.

    • Introduces self to patient, explains the nature of the trip and escorts the patient by wheelchair or stretcher to and form the nursing unit to the physical therapy division, using appropriate hand-off communication.

    • Demonstrates sensitivity to differences in religious, ethnic and cultural practices.

    • Maintains control of each patient's chart and only releases each chart to the physical therapist or physical therapist assistant treating the patient. Once a patient is escorted back to a nursing unit, the patient's chart is placed at the nurse's station and the nurse responsible for the patient is notified of the patient's return to the unit.

    • Adheres to the policy and procedure for the transport, storage and repair of wheelchairs for offward inpatients scheduled for physical therapy.

    • Communicates appropriately with the nurses throughout the hospital and with the rehabilitation health care team to ensure that patient transport or scheduling problems are resolved.

    • Answers inquiries from the physical therapy staff about patients who are unable to attend their scheduled therapy sessions.

    Minimum Qualifications

    Elementary school graduation or equivalent ability to effectively communicate with patients; Ability to keep simple clinical records.

    Department Preferences

    • Knowledge of using proper body mechanics when transferring patients to and from the physical therapy gym.

    • Knowledge of protocols for cleaning wheelchairs and stretchers with either purple super Sani-Cloth germicidal wipes or Sani-Cloth bleach wipes.

    • Knowledge of matching an appropriate size wheelchair to the patient being transported with or without an oxygen carrier.
