    Job Description Summary:
    Performs patient care by assessing the patient's physical and functional abilities.

    Based on problems determined, the physical therapist establishes and carries out a plan of care which includes patient/family instruction, discharge planning, documentation on the medical record, and communication with the physicians and other medical staff.

    This position helps patients to achieve their maximum level of physical functioning. Clinical instructor for students, and actively participates in department functions.


    Minimum Qualifications:

    Bachelor's Degree:

    Physical Therapy (Required)DL-HC - Driver's License only if in a Home Care department - Department of Motor VehiclesDepartment of Motor Vehicles, LIC - Licensed in Field - State of OhioState of Ohio, PT- Physical Therapist License - American Board of Physical Therapy SpecialtiesAmerican Board of Physical Therapy SpecialtiesState Driver's License.

    Licensed in the State of Ohio. Basic computer skills.

    Entry level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical Experience .
