    Job Description: Wraparound Services Specialist

    Main Responsibilities

    • IDentify and address client and family needs by providing direct service, case management or intensive in-home family preservation/reunification services as appropriate.
    • Coordinate and facilitate Family Team Conferences to develop an appropriate plan based on the needs of the families and oversee compliance with Wraparound services, values and standards.

    Service Delivery and Treatment

    • Oversee services and ensure treatment is delivered from a strength-based framework customized to meet individualized needs of consumers.
    • Provide case management services, monitor appropriateness of services and participate in clinical review of clients.

    Intensive In-home Family Preservation/Reunification

    • Assess the needs of children and families, including child safety factors and family functioning.
    • Conduct in-home case coordination and counseling visits at the frequency and intensity consistent with their higher level of need.

    Additional Responsibilities

    • Contribute to a positive, engaging work environment by developing a strong knowledge base and staying current on job-related issues and trends.
    • Maintain regular contact with clients, family members and other professionals to advocate for services and address issues related to the client's plan of care.

    Job Qualifications

    Education, Licenses & Certifications

    • Bachelors Degree in Social Work or Human Service related field.
    • A valid Florida drivers license.

    Experience

    • Two years or more experience working with behavioral health/child welfare/juvenile justice and severely emotionally disturbed children.

    Knowledge, Skills and Abilities

    • Knowledge of family/social services, child welfare, community resources.
    • Skills and proficiency in assessment skills, intervention strategies, verbal and written communication, computer systems and MS Office.
