- Conduct initial nursing assessments to determine eligibility for hospice services promptly after referral.
- Identify and reassess the physical, psychosocial, and environmental needs of patients and their families regularly.
- Develop and coordinate the care plan while maintaining accurate clinical notes.
- Document issues, goals, interventions, and patient/family responses to hospice care effectively.
- Collaborate with patients, families, physicians, and the care team to deliver daily care.
- Minimum of one year of clinical experience.
- Active RN license in the state of practice with at least one year of clinical experience.
- Current CPR Certification.
- Valid driver's license, reliable vehicle insurance, and access to transportation.
- In various states including AL, AR, AZ, CA, CO, and more - there are no extra state-specific mandates to meet.
