    Job Summary

    • Specialty: Physical Therapy

    This role involves performing home health visits for adult and geriatric clients on a per visit basis. The ideal candidate will have experience with current home health and Oasis protocols.


    About the Position


    • We are seeking an experienced therapist who can work independently to manage client care plans.

    The successful applicant will be responsible for assessing clients' needs, developing individualized care plans, and implementing appropriate interventions. Strong communication skills are essential for effective collaboration with healthcare teams and family members.


    Key Responsibilities:


       
    • Maintain accurate records of client interactions, progress notes, and treatment outcomes using electronic medical records (EMR).
      •  
    • Collaborate with multidisciplinary healthcare teams to develop comprehensive care plans that meet the unique needs of each client.
      •  
    • Educate clients and their families about rehabilitative care techniques to enhance independence at home.

    The selected candidate will possess excellent time management skills due to frequent travel between designated territories in West Baltimore County. A valid Maryland license as a Physical Therapist-PT is mandatory.
