Bi-Lingual Case Manager

• Communication and relationship building with client and all collateral partners connected with each client (schools, funding partners, therapists, agencies, clinicians, supervisor, foster homes, etc.).
• Setting limits for youth, development of interventions, setting goals toward individual successes in conjunction with client's guardians, therapist and other providers.
• Initiate and monitor external recommendation processes and treatment planning for clients in case load.
• Ensure compliance with policies and procedures, including all contract sources.
• Interact in a timely and effective reporting as directed for computer reports, tracking, paper reports and the like in accordance with agency protocols.
• Participate in on-call services as needed.
• Represent Agency on task forces and community boards regarding FBS interests.
• Possess and exercise strong interpersonal skills throughout all levels of Agency.
• Attend meetings throughout Agency and with collateral partners.
• Keep abreast of trends, significant events in the field of Human Services and Case Management.
• Analyze situations, identify problems, identify and evaluate alternative courses of action.
• Plan and organize work effectively and ensure its completion.
• Use discretion and individual judgment with agency under the guidance and direction of supervisor.
• Maintain highest level of confidentiality.
• Responsibly, accurately, and completely document all work and share as appropriate within FBS and with collateral partners.
• Other duties as assigned related to these primary responsibilities.

?Verification of adequate personal auto insurance on the vehicle(s) to be used, including, at minimum, Optional Bodily Injury to Others (coverage part 5 on a Mass Auto policy) limits of $100,000 per person/$300,000 per accident, and Damage to Someone Else's Property (part 4) limits of at least $100,000
