    Summary


    • The Post Acute Care Coordinator is responsible for identifying early in the hospital stay patients appropriate for lower level of care facilitating smooth transition from acute to PAC care settings. This position reports to the Director of In-Patient Care Management. As a critical member of the care management teams, the Coordinator works closely with hospital and Acute Rehabilitation, LTAC, swing beds, skilled nursing facilities and nursing home discharge planners, physicians, palliative team members, social workers, case managers, management, inpatient units and education staff to facilitate smooth hand-off of care, assisting with identifying high risk patients for readmissions and referrals to Post Acute Care Providers. The Coordinator will develop and maintain new and current relationships with various areas of Mosaic Life Care Hospital, Clinics and PAC providers. This position is employed by Mosaic Life Care.
    Duties


    • Assesses potential patient's condition and past medical history and obtains appropriate documentation to support the appropriate post-acute care eligibility.
    • The evaluation should consider what the patient's care needs will be immediately upon discharge. If returning home consideration should be given to self-care assistance both equipment and family to support and if not available, then referral to higher level of care.
    • Provides an overview of the appropriate post-acute care service.
    • Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations. Complies with all Care Management and Mosaic Life Care policies and procedures.
    Qualifications


    • Graduate from school of nursing is required. BSN is preferred.
    • RN - Registered Nurse - State Licensure And/Or Compact State Licensure depending upon designated work location within 90 days of hire is required. Clinical\BCLS - Basic Life Support required within 90 days. ACLS - Advanced Cardiac Life Support required within 90 days. IF working in an inpatient area in which ACLS is required within 90 Days
    • 3 years of Clinical experience in post-acute care required.
