- Provides age-appropriate therapeutic interventions as instructed by the physical therapist across multiple care settings.
- Assesses patient response to treatment and offers feedback to the team.
- Documents the progress and ongoing treatment needs of patients.
- Participates in preparing for surveys.
- Collaborates with the physical therapist to innovate patient and family education methods.
- Supports the physical therapist in training team members on physical therapy concepts and skills as required.
- Contributes to providing cost-effective care.
- Reports resident complaints to the Rehab Program Director or Director of Health Services.
- Reports any incidents or suspicions of resident abuse promptly.
- Willingness to travel to different locations as needed.
- Adheres to all resident rights according to company policy.
- Maintains professional competencies for job duties.
- Graduate of an APTA approved associate degree program or equivalent with a valid license to practice.
- Experience working with adult and geriatric populations is preferred.
- Must possess a valid state license and current CPR Certification.
- Two (2) years' experience in healthcare, senior living, or long-term care settings preferred.
- Proficient in Microsoft Office applications.

Responsible for delivering therapeutic interventions, under the guidance of a physical therapist, in an interdisciplinary setting that aligns with professional standards and ethical practices.
Demonstrates responsibility for program enhancement, quality improvement, creative problem-solving, and productivity optimization in a collaborative interdisciplinary environment.
ROLE AND RESPONSIBILITIES:
QUALIFICATIONS AND EDUCATION REQUIREMENTS:
PHYSICAL REQUIREMENTS: Sitting, standing, bending, reaching, stretching, stooping, walking, and lifting up to 50 lbs. Must have sensory capabilities and operate office equipment effectively. Good verbal and written communication skills are essential.
