    St. Lukes home health team of skilled professionals provides comprehensive health care in the warmth and comfort of the patients home. From newborn to seniors, we offer the latest home health techniques and technology to people at all ages and stages of life. Dedicated to the continuum of care as patients transition from inpatient to home to outpatient settings for care. Our home-based services team (NP, RN, MA, MSW) also assists with oversighting transitions. Close working relationship with St. Lukes Hospice team.

    The Home Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care from admission to discharge for an assigned case load. This is a direct care position providing in home care that requires traveling within the Treasure Valley area.

    What You Can Expect:

    • Full Time Days (Thurs-Sun from 8:00am-6:00pm) with rotating evening call
    • Manageable case-loads (Daily visit requirements below state and national average
    • Pay for drive time)
    • Mileage reimbursement (Geographical Area: Treasure valley team has dedicated team zones to cover the following counties in Idaho: Ada, Boise, Canyon, Gem, Owyhee, Payette, Washington)
    • Applicable Differentials/On-call Pay
    • Cross-training opportunities in home infusion/transitions of care

    Qualifications:

    • Education: Degree from an Accredited Program
    • Experience: 3 years relevant experience
    • Licenses/Certifications: Valid license to practice audiology, physical, occupational or speech therapy in state of practice
    • Valid Driver's License and proof of Auto Insurance
    • Preferred Qualifications:
      • Occupational or Physical Therapist
      • Home Health Experience
