An established practice serving Waterbury, CT and surrounding communities is seeking a Nurse Practitioner (APRN) or Physician Assistant (PA-C) to join their team on a full time, permanent basis. Monday-Friday. No weekend requirements. Must be able to work autonomously.

· Provide outpatient primary care services
· Assess patient health through interviews and physical examinations
· Provide counsel and medical treatment to patients
· Educate patients on overall wellness and health maintenance
· Develop and implement patient management plans
Qualifications:
· Active CT license
· Ability to build rapport with patients
· Excellent written and verbal communication skills
· Nurse Practitioner, Physician Assistant experience.
