Avera is currently looking for Registered Nurses (RN) Pharmacy Case Manager

Job Summary

The pharmacy case manager plays a crucial role in coordinating the pharmacy care plan in collaboration with clinical pharmacists, physicians, and payors. They ensure proper utilization of specialty medications to enhance patient adherence, clinical outcomes, and meet payor guidelines. Conducts regular assessments following therapy management protocols and collaborates with the care team to optimize patient outcomes. Acts as a liaison with physicians and other referral sources to ensure seamless patient care.

Education and/or Experience

• Preferably a graduate from a CCNE or NLN accredited nursing program, although graduates from an approved nursing program are also considered.
• Preferred bachelor's degree in nursing.

Licensure/Certification/Registration

• Must hold a current and unrestricted registered nurse license in South Dakota or meet requirements for multi-state licensure.
• Required to attain unrestricted registered nurse licensure in Minnesota within 90 days.
• Specialty certification is a plus.
• Membership in a professional organization is encouraged.

Work Schedule

• Monday to Friday
• 80 hours per 2 weeks

Pay

The hourly rate for this role is dependent on experience and falls within the range of $44.25 per hour.
