    Description

    At WelbeHealth, every participant in our program is guided by our interdisciplinary team (IDT) - made up of clinical and non-clinical members who collaborate to offer quality patient-centered care.

    The Physical Therapist plays a crucial role in the IDT, actively involved in care planning and working closely with the rest of the clinical team.

    The primary focus of the Physical Therapist is to coordinate and deliver restorative and rehabilitative Physical Therapy services to participants both at the Center and in their homes.

    Working alongside the Primary Care Physician, rehabilitation team, and other IDT members, the Physical Therapist strives to enhance participant independence, safety, and function development or restoration.

    • Conduct Physical Therapy assessments in our facility and at participants' homes to determine their care needs, preferences, and goals
    • Participate in IDT meetings to develop care plans for participants
    • Provide and document Physical Therapy interventions as per physician orders and/or agreed care plans
    • Offer exercises, muscle re-education, gait and functional training, as well as various treatments
    • Supervise Therapy Assistants and provide necessary training for Functional Maintenance Programs
    • Train staff, participants, and caregivers in the use and maintenance of therapeutic equipment


    • Bachelor's Degree in Physical Therapy plus five (5) years of experience
    • Master's Degree or Doctorate in Physical Therapy preferred
    • Valid California Physical Therapy License or eligibility
    • Two (2) years of experience in a clinical setting with frail or elderly populations preferred
    • International physical therapy degree must be evaluated to the US equivalent
    • CPR certification preferred

    Salary range for this role is $99,309.58—$119,171.49 USD annually + Bonus + Equity. Actual pay based on experience and qualifications.
