    Women & Infants Hospital is seeking a Per diem Nurse Practitioner ( PNP or FNP) to join our Transition Home Health team. This program helps at-risk pre-term infants transition from the hospital to their homes which will improve infant health and reduce the risk of rehospitalization.

    Opportunity Highlights:

    • Home visits (3-6 per week) and Office visits with medically complex patients aging from newborn – 1 year old
    • Team consists of 3 neonatologist, 1 nurse practitioner and social workers
