PRN Physical Therapist - Pinnacle Home Care, Florida

Role Summary: As a Physical Therapist, you'll be an essential member of our healthcare team, providing Physical Therapy services in compliance with all applicable laws and standards. You'll work under the direction of a physician and nursing supervisor to deliver skilled Physical Therapy care to our clients in their homes.

Key Responsibilities:

Conduct initial and ongoing assessments, including OASIS assessments.
Develop and revise treatment plans with physician approval as necessary.
Participate in the development and implementation of interdisciplinary client care plans.
Instruct and counsel clients and families on physical therapy programs.
Supervise Physical Therapist Assistants in accordance with regulations.
Communicate with physicians and team members regarding client status and care.
Document clinical/progress notes promptly and accurately.

Qualifications:

Graduate from an accredited Physical Therapy program.
Valid Florida PT license.
Current BLS Certification.
Valid driver's license with reliable and insured vehicle.
Preferably one year of experience as a Physical Therapist, with home healthcare experience beneficial.
Proficiency in electronic medical records preferred.
Ability to work independently with excellent observation, judgment, and communication skills.
