    The Community Care Manager, with a panel assigned leadership responsibilities:
    • (I) performs home-based clinical services injections specimens laboratory evaluation IV fluids ordered Primary Provider gives ongoing education help patients meet goals facilitates conversations life targets family HIPAA Representative supports staff specialists including Case Managers caregivers families agents adhering workflow templates care management strategies ensuring updates close gaps workflows submitted comply requirement.
