Description

Physical Therapist in Home Health

The Physical Therapist in Home Health evaluates patient care needs related to functional status and other medical conditions, determines treatment plans, performs interventions, and monitors progress. Responsibilities include:

Evaluating patient's functional status and physical therapy needs
Providing services within scope of practice and collaborating with healthcare team
Observing and reporting patient's response to treatment
Treating patients to relieve pain and restore function
Instructing healthcare team and patients on plan of care and progress
Visiting patients in service area, completing progress notes, and reporting outcomes
Participating in staff conferences, committees, and in-service education
Supervising home health aides and PTA as required
Adhering to therapy standards of practice and maintaining licensure
Rotating weekends to provide services
Preparing discharge and summary reports
Ensuring education for clinical knowledge and skills
Performing other assigned duties as required

Licensure Requirements

Current Physical Therapy licensure in state of Florida
Current CPR certification
Must have a current driver's license, vehicle insurance, and access to a reliable vehicle
