    CARE SPECIALIST

    Upward Health is a multidisciplinary medical group offering comprehensive 24/7 care in the comfort of patients' homes. Our team addresses physical, behavioral, and social health needs at the right time and place. Each member of our team, from providers and nurses to Care Specialists and support staff, is motivated to enhance the well-being of our patients.     Responsibilities

    • Conduct outreach to patients via various methods such as phone calls, in-person visits, and mailings.

    • Enroll patients in the program and gather essential data through continuous interactions.

    • Meet patients in their homes or communities to assess needs and set health goals.

    • Coordinate in-home provider visits and ensure proper care coordination.

    • Support patients in achieving healthcare objectives and Quality measures.

    • Collaborate with a multidisciplinary team to meet organizational goals.

    • Record vital signs and health information accurately in electronic medical records.

    • Analyze patient data to identify needs and treatment goals.

    • Educate patients on medical information and facilitate care transitions.

    • Build rapport with patients using motivational interviewing techniques.

    • Conduct in-person patient appointments and follow-up visits.

    • Document patient encounters meticulously and prepare reports as necessary.

    • Participate in team meetings, huddles, and clinical rounds.

    • Perform other duties as required.

    Requirements

    • Minimum of 2 years experience in a related role such as Community Health Worker or Medical Assistant.

    • High school diploma or GED required.

    • Valid driver's license and reliable transportation.

    • Experience in Chronic Care Management or working with chronically ill patients.

    • Familiarity with community resources and the ability to complete necessary training.

    • Strong organizational and time management skills.

    • Excellent communication and critical thinking abilities.

    • Proficiency in electronic medical record systems.
