Job summary
The Infectious Disease Outreach Navigator will provide comprehensive community-based outreach, education and promotion of HIV and HEP-C rapid testing prevention and information regarding PrEP related services available through Carolina Health Centers.This includes patient education and outreach in the areas of HIV/HEP-C prevention testing treatment linkage to care once connected with system of care assist patients effectively engage healthcare system promote retention improved health outcomes.
Provide comprehensive community-based outreach education promotion HIV testing HIV prevention PrEP related services available health center accurately document all patient interactions electronic health record strengthen community partnerships identify high-risk populations ensure culturally appropriate approaches engage communities risk acquiring HIV including partnerships opioid treatment programs medication-assisted treatment providers organizations providing counseling behavioral therapy SSPs housing programs faith-based organizations community centers local health department State Disease Intervention Specialists DIS AIDS service organizations community-based service organizations non-traditional communities Alcoholics Anonymous Domestic Violence Groups Homeless Shelters etc assist people at risk acquiring HIV completing HIV testing engaging re-engaging system care including ,HIV prevention services medical services as appropriate educate one-on-one patients ,community members promote knowledge teaching basics HIV/Hepatitis C, help improve communication medical providers educate living living viral load CD4 problem-solving individual adherence plans support access adherence through education counseling organize participate events attract enroll raise awareness HIV PrEP post-exposure prophylaxis PEP, provide HIV Prevention Education.
